1. The school employer will provide for custodial employees who are
regularly employed for twenty (20) hours or more per week a Group Life,
Dental, Vision, Prescription Drugs, Hospitalization, and Major Medical
Insurance. The employer will pay the following amount per year:
The plan will provide for a dependent age of 19 (except for full
time students in which case the age will be 23), for a prescriptive drug
plan with a $5.00 CoPay amount for each prescription and transplant coverage.
The Union agrees to make further plan design changes to prevent the Board
from contributing more than the aggregate health insurance contribution
for this unit of $237,000 if the employer continues with Accordia as the
insurance carrier. The total health insurance contribution by the employer
in policy year 1996-97 will be $237,000 if the employer continues with
Accordia as the insurance carrier. The Union agrees to make further plan
design changes to prevent the Board from contributing more than the aggregate
health insurance contribution for this unit of $227,000 if the employer
changes to United of Omaha as their insurance carrier. The total health
insurance contribution by the employer in policy year 1996-97 will be $227,000
if the employer changes to United of Omaha as their insurance carrier.
If the cap is exceeded in the 1996-97 policy year, the unit may modify
the insurance plan, absorb the cost, or seek coverage under another arrangement.
The cap on the employer's insurance contribution will be $231,540 for the 1997-98 insurance policy year. The cap for the 1998-99 insurance policy year will be reopened for negotiations in December 1997.
If additional employees are added to the custodial bargaining unit, the insurance cap will be increased by the amount of the employer's cost of the insurance for the additional employees.
2. If a custodial bargaining unit employee drops coverage under the group health insurance plan for the plan year, the employee shall receive an annual $250 payment in the second pay in July of each plan year. If an employee changes from a family plan to a single one for the plan year, the employee will receive $250.00 on the second pay day in July of the year in which the change is made.
3. LTD
The employer will provide a group long term disability plan for custodians
and contribute $70.00 annually to the premiums for such plan, effective
July l, 1986. Effective September 1, 1989, there will be an improvement
in coverage.
4. Section 125 Plan
Within sixty (60) days of the ratification of the agreement. The employer
will make available to employees a Section 125 Plan.
